An applicant for a license herein shall submit his application to the chief of police, which application shall be under oath and shall include, upon appropriate forms to be furnished therefor, the past criminal record, if any, of the applicant, together with fingerprints, a photograph and physical description of the applicant, together with identity of all persons financially interested in the business or profession for which license is sought.
