Application for a grading permit, not exempted by Section 9B-4, shall be submitted to the environmental review board for evaluation and shall include:

(a) A description of the land on which the proposed work is to be done by lot, block, tract and street address or similar description;

(b) Site plans showing property boundaries, location of existing structures, adjoining streets, easements, access, drainage features, existing topography, final proposed grades, plans for disposition of surface water run-off and erosion control;

(c) Quantity and type of material to be graded, excavated or filled;

(d) Purpose of grading, excavation or fill;

(e) Source of materials used for fill or location to which excavation material will be moved;

(f) A statement containing starting and completion dates;

(g) A statement describing routes for hauling materials;

(h) Name and address of the person responsible for the correctness of work done under the permit and for requesting inspections;

(i) Soils report when required by the director of public works or the environmental review board;

(j) A fee of thirty-five dollars for environmental review. (Ord. No. 1319,§ 5.)
